Will AI replace physical therapists?
No. Physical therapy scores 76.2/100 on the AI Resistance Index (Conditionally Viable), one of the strongest results in the index: the work is hands-on, licensed and built on trust with patients, and BLS projects much faster than average growth. AI helps with documentation, scheduling and exercise tracking, not with the treatment itself.
Higher is safer. Scored on six structural dimensions, weighted as shown below.
How the score breaks down
Each dimension is rated 1 (AI holds every advantage) to 5 (strong structural protection). A rating's contribution is (rating − 1) ÷ 4 × weight, so a 1 contributes nothing and a 5 contributes the full weight.
| Dimension | Weight | Rating | Points |
|---|---|---|---|
| Automation Replaceability | 25% | 4 / 5 | 18.75 |
| Regulatory & Liability Friction | 20% | 4 / 5 | 15 |
| Human Judgment Density | 15% | 4 / 5 | 11.25 |
| Physical-World Coupling | 15% | 5 / 5 | 15 |
| Trust & Relationship Lock-In | 15% | 4 / 5 | 11.25 |
| AI Capital Intensity | 10% | 3 / 5 | 5 |
| Total | 100% | 76.2 |

Why each dimension scores the way it does
Rated 4. Assessing how a person moves, hands-on therapy and adjusting treatment as a patient progresses are physical, individualized work that AI cannot do. What AI does touch is the paperwork: notes, billing codes and home exercise programs, which take real time but are not the core of the job.
Rated 4. Every state requires a licence, which means a Doctor of Physical Therapy degree and a qualifying exam, plus continuing education. Treatment carries professional liability, and insurers pay for care delivered by licensed clinicians.
Rated 4. Each patient is different: injuries, conditions, pain, motivation and setbacks all change the plan. BLS lists reviewing histories, assessing movement and adjusting treatment among the core duties, and each one needs clinical judgment.
Rated 5, the strongest dimension. Treatment means hands on a patient: manual therapy, guided exercise and equipment. Telehealth helps with follow-ups, but the core work happens in the room.
Rated 4. Recovery takes weeks or months of working closely with one therapist, and patients and referring doctors return to clinicians they trust.
Rated 3. Replacing physical therapy would need advanced robotics and clinical validation, not just software. The AI that exists here today supports therapists rather than substituting for them.
Which tasks AI takes first
BLS describes the core duties of physical therapists. Sorted by how exposed each is to AI today:
Most exposed to AI
- Writing visit notes and documentation, which AI scribes now draft
- Building and tracking home exercise programs with apps
- Scheduling and insurance paperwork
Most durable
- Assessing how patients stand, walk and move
- Hands-on therapy, stretching and guided exercise
- Creating and adjusting individualized care plans
- Educating patients and families through recovery
The labor market today
| Median annual pay (May 2025) | $102,760 |
| Jobs (2025) | 283,700 |
| Projected growth, 2025–35 | +12% |
| Openings per year, on average | ≈13,400 |
| Typical entry-level education | Doctor of Physical Therapy (DPT) |
BLS projects 12% growth for physical therapists from 2025 to 2035, much faster than average, an increase of about 33,800 jobs. All states require a licence.
Growth and resistance point the same way here. Demand is rising and the core of the work is physical, licensed and personal, which is why physical therapists score 76.2, among the strongest jobs in the index.
How to raise your resistance
- Let AI take the notes. AI documentation tools give back hours each week. Use them to see more patients or spend more time with each one.
- Specialize. Board-certified specialties such as orthopedics, neurology, sports and geriatrics deepen the judgment and trust that protect the role.
- Own the patient relationship. Patients who trust you come back and refer others. That loyalty matters more than any technology change.
- Lead the digital follow-up. Apps and telehealth will grow around treatment. The therapist who designs and supervises them keeps control of the care.
This score rates the occupation as a whole. Your exposure depends on what you actually do all day. The free Job Safety score asks about your specific role and day-to-day work, then rates your role on the same six dimensions in about a minute.

Will AI replace physical therapists?
No. Treatment is hands-on, licensed and individualized, which is why physical therapists score 76.2/100, one of the strongest results in the index. AI mainly helps with documentation and exercise tracking.
Is physical therapy a good career for the future?
BLS projects 12% growth from 2025 to 2035, much faster than average, with a median pay of $102,760 in May 2025 and about 13,400 openings a year.
What part of physical therapy does AI affect?
Mostly paperwork: visit notes, billing and home exercise programs. The core work of assessing and treating patients rates 5 of 5 for physical-world coupling.
